Preheat the oven to 350°F (175°C) and line a baking sheet, or lightly grease a loaf pan.
In a large bowl, combine the ground beef, breadcrumbs, milk, egg, grated onion, ketchup, salt and pepper. Mix gently until just combined and do not overmix.
Shape the mixture into a loaf and place it on the baking sheet or into the prepared pan.
Spread a thin layer of ketchup over the top, right to the edges.
Bake for 55 to 60 minutes, until the internal temperature reaches 160°F (71°C).
Let the loaf rest for 10 minutes before slicing.
